Insurance
:- Our practice participates with most insurance policies offered
in this area, but it is still your responsibility to make sure we
are participating providers. |

PCP/Primary
Care Physician :- Please make sure that we are listed
as your Primary Care Physician with any HMO/POS product. If our providers
are not listed on your insurance card you will be asked to sign a
waiver and will be held 100% responsible for the bill if insurance
payment is denied. |

Referrals
:- Please allow 5 days for non-urgent referrals to specialists. The
pre-certification process for CT, MRI and nuclear testing will take
longer. This process is dependent on your insurance company’s
response time. You will be notified by phone call or mail once the
referral or pre-certification is in place. |

Pre-Certification
:- Many insurance companies require that tests be pre-certified with
the insurance company before they are even scheduled. Please realize
our office is working on your behalf but again be aware that it may
take up to 7-10 days to get approval. Our office will notify you once
the pre-certification is received and the test will be scheduled.
