| Our office will be happy to file a claim
with your primary insurance carrier, but the patient is ultimately
responsible for the payment regardless of insurance coverage. Our
office does not bill secondary insurance. If the insurance company
does not pay within 60 days we will send in the claim a second time.
If the claim is not paid or some type of denial received within 90
days the bill will be turned over to you. |

Self-Pay : We give a 20%
discount on all services if the patient pays cash/credit card/check
at the time of service. |

Co Pays : Co pays are
due at the time of service. Additional fees may be charged if the
patient must be billed for their copy. |

Payment Plans : We are
more than happy to set up a payment plan if it is necessary, but you
need to notify our office and set it up. |

Finance Charge : Our office
will charge a finance charge of 1.5 % APR on any balance older than
60 days |
|
Collection Agency Assignment
: All patient balances aged 120 days or older will be turned over
to a collection agency and the patient will be discharged. |
